For PayPal you can use Visa, American Express, MasterCard, Discover, E-checks, Bank Transfers, or your personal PayPal account. Prior to using the PayPal link call our office and speak with our office manager to confirm the cost of your deductible or payment. Personal Training Purchase

3 Initial Personal Training Sessions (for first time clients only) $50.00 total

1 Personal Training Session $60.00 total

5 Personal Training Sessions (5 x $55.00) $275.00 total

10 Personal Training Sessions (10 x $50.00) $500.00 total

If you maintain your gym membership your sessions will expire after 365 days. If we purchase a temporary membership for you the sessions you purchase will expire in the following way:

10 session package purchase expires in 90 days

5 session package purchase expires in 30 days
1 session purchase expires in 7 days
3 initial session package expires in 30 days if you maintain your membership and 7 days if we purchase a temporary membership



Insurance

Understanding your insurance benefits can be quite confusing. Orthopedic Physical Therapy and Wellness Inc. is here to help you understand them in the simplest way possible. Many people are hesitant to use their healthcare benefits because they are afraid of the tedious task. The administrative process can be very technical. Dealing with paper work, claims filing, denials, appeals and hassling with insurance representatives is irritating.

For your convenience, Orthopedic Physical Therapy and Wellness Inc. will verify your insurance benefits and talk to you candidly about them. We will never hide anything and will always seek to clarify misunderstandings about your coverage and benefits. Your utmost confidence in our practice is our concern. Our office staff will gladly file the Physical Therapy claim on your behalf to your insurance provider. You only have to worry about one thing, getting better.

OPTW Inc. is a Medicare participating Provider. For patients who have private insurance, we accept most PPO Health Plans. Most PPO health plans usually have in-network and out-of-network benefits. Regarding other insurance companies Orthopedic Physical Therapy and Wellness Inc. is an out-of-network provider.

We will happily work with your insurance company, verify your benefits, and discuss your co-pay with you. Co-pay should be paid at the time services are rendered. Orthopedic Physical Therapy and Wellness Inc. accepts cash, check, money order, PayPal, and major credit cards. Co-pay depends on your healthcare insurance plan benefits. OPTW Inc. accepts the following insurance.
